Patient Pathway is a new rapidly-growing team within DaVita, the nation's largest independent provider of kidney dialysis.
We are in search of a strong leader to manage and support the geographical areas of Mississippi, Florida, Tennessee, Kentucky, Georgia, Alabama, South and North Carolina .
The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive:
Implement DaVita's Patient Pathway Program
Travel to new program hospital partner locations to initiate the Patient Pathway program
Meet face to face with hospital partners to discuss current discharge processes
Structure, introduce and train hospital partners on the Patient Pathway program
Identify areas of improvement in the discharge process in individual local markets
Conduct regular update check-ins with hospital partners and Patient Pathway management before, during, and after implementation
Conduct Local Market Education, Awareness and Strategy
Conduct meet-and-greet training sessions with DaVita operations leadership to introduce Patient Pathway
Update local operations leadership on the progress and impact of the Patient Pathway program
Discuss ongoing strategies for each market with Patient Pathway and field management
Perform Supervisory Responsibilities
Provide leadership to direct reports; coaching, mentoring, training as required or needed
Manage Dialysis Coordinators within the Patient Pathway program including but not limited to teammate relations, fostering a positive work environment, conducting annual performance and salary reviews, performance management, recruitment, retention, terminations
Conduct regular update calls and operating reviews with each direct report
Manage the hiring and onboarding process for Dialysis Coordinators in assigned territory
Know, understand, implement and follow DaVita Teammate Guidelines and employment policies and procedures
Schedule, organize and conduct new Growth teammate training, including both classroom training and on-site shadowing
Regularly assess training content, making recommendations and modifications when necessary
Dialysis Coordinator Coverage Assistance
- Provide coverage assistance at Patient Pathway partner hospitals as needed (including but not limited to covering during teammate PTO leave or position vacancies)
Report on Program
Analyze Patient Pathway reports for assigned territory including but not limited to patient volume for each Dialysis Coordinator and length of stay metrics for territory and individual hospitals
Analyze performance of each market and make recommendations on future action plans based on the analysis
Provide management team with monthly updates regarding the above metrics
Analyze and report on P/L and complete budgets
Analyze and Initiate Growth Initiatives
Analyze, develop, and implement new ways for the company to differentiate itself in the marketplace
Develop in-depth knowledge of the company's service offerings, markets, current market share within individual markets, market competition, and strategies to secure new business
Participate in special projects, as needed

The right hire will have these requirements:
Travel Required: up to 75 %
Bachelor's degree is required
Minimum of one (1) years’ experience demonstrating strong understanding of commercial insurance plans and benefits (PPO, HMO, POS, EPO, Indemnity) as well as government insurance programs (Medicare, Medicaid) preferred
Minimum of two (2) years' management experience required; preferred management of multi-site or multi regions is highly preferred
Remote teammate/ employee management experience preferred
Healthcare industry management experience preferred
Two (2) years' experience providing services or products to hospitals preferred
Experience with P/L and Budgets highly preferred
Intermediate computer skills and proficiency in MS Excel, Word, Outlook, and PowerPoint required
Home office with Internet connectivity at minimum of 1MB upload and 1MB download speed required * will work remotely as well *
